Children and Young People's Strengths and Difficulties Questionnaire (SDQ)
The SDQ is a brief behavioural screening questionnaire about the looked after children/young people aged 4-16 years. It exists to meet the needs of the young people in our care.
The SDQ asks whether you think the young person has any issues, and if so, enquiries further about how chronic, distressing they are, whether they cause any social impairment or are a burden to others. You will be asked a series of questions and asked to rate them 'not true', 'somewhat true' or 'certainly true'.
This provides useful additional information for clinicians to determine service use such as CAMHS and other specialist services for children with emotional and behavioural difficulties.
If you have any concerns or need to discuss how to complete the form please speak to your supervising social worker in the first instance.
If you do not think you have received one or have one to return please contact the Through Care Team via 0115 804 1224.
